PURSUANT TO THE AGREEMENT BETWEEN THE U.S. GENERAL SERVICES ADMINISTRATION AND OHIO VALLEY GOODWILL INDUSTRIES, AND IN ACCORDANCE WITH FAR 52.222-43 FAIR LABOR STANDARDS ACT AND THE SERVICE CONTRACT ACT - PRICE ADJUSTMENT, THE ABOVE REFERENCED CONTRACT IS MODIFIED AS FOLLOWS: OPTION LOT FOUR (4) IS HEREBY EXERCISED IN ACCORDANCE WITH THE CONTRACT CLAUSE ENTITLED FAR 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT. THIS MODIFICATION NO. PO04 EXTENDS THE CONTRACT FOR THE PERIOD JUNE 1, 2012 THROUGH MAY 31, 2013, WHICH EQUATES TO $102,428.72 PER MONTH OR $1,229,144.64 ANNUALLY. YOUR PROPOSAL SUBMITTED 03/20/2012 IS HEREBY ACCEPTED. ALSO, IN ACCORDANCE WITH THE CONTRACT CLAUSE ENTITLED FAR 52.222-43 FAIR LABOR STANDARDS ACT AND SERVICE CONTRACT ACT--PRICE ADJUSTMENT (MULTIYEAR AND OPTION CONTRACT), THIS MODIFICATION INCORPORATES WAGE DETERMINATION NO. 2005-2413, REVISION NUMBER 11, DATED JUNE 13, 2011. THIS WAGE DETERMINATION, AN EXTRA COPY OF WHICH IS ENCLOSED, WILL BE EFFECTIVE ON THE ANNIVERSARY DATE OF THE CONTRACT AND SHOULD BE DISPLAYED IN A CONSPICUOUS PLACE AVAILABLE TO ALL EMPLOYEES PERFORMING WORK UNDER THE CONTRACT.
