THE A/E INDICATED A CORRECTED SEQUENCE AND THIS CO AUTHORIZES THE CONTRACTOR TO MAKE THE CHANGES. 2. THE ORIGINAL PLANS INDICATED USING THE EXISTING CHILLED WATER MAKEUP IN THE MEDICAL CENTER. SINCE THE NEW BUILDING IS ABOUT 30 FEET TALLER THAN THE EXISTING BUILDING THERE IS INSUFFICIENT WATER PRESSURE TO FILL THE CHILLED WATER SYSTEM. THIS CO AUTHORIZES THE CONTRACTOR TO INSTALL A NEW CHILLED WATER MAKE UP WHICH WILL FILL THE SYSTEM OFF THE DOMESTIC BOOSTER SYSTEM.

THE MEDICAL CENTER LATER REQUESTED THAT THE ISOLATION ROOM BE INCLUDED AS PART OF THE BED TOWER PROJECT BECAUSE THE MC WAS UNABLE TO PERFORM THE WORK IN RELOCATING THE ISOLATION ROOM AS INITIALLY AGREED THIS CHANGE ORDER DEFINITIZES CO-02V TO INCLUDE THE ISOLATION ROOM CHANGES AND TO ADJUST THE COST TO PERFORM THE WORK.

A NO VALUE EXCHANGE OF WORK BETWEEN THE VAMC AND THE CONTRACTOR. THE CONTRACTOR CONSTRUCTED A TEMPORARY WAITING ROOM IN EXCHANGE FOR THE VAMC CONDUCTING THE ABATEMENT OF THE RENOVATION AREA. SOME WALLS OF THE ORIGINAL DESIGN HAVE ALREADY BEEN INSTALLED AND WILL NEED TO BE REMOVED TO ACCOMMODATE THE NEW DESIGN.

THE SUMP PUMPS IN THE ELEVATOR PITS WERE PERMANENTLY INSTALLED AND WERE NOT REMOVABLE. 2. THERE WAS NO LIGHTING INSTALLED AT THE TOP OF ELEVATOR 15. 3. A GFI WAS MISSING IN THE ELEVATOR PIT OF THE SERVICE ELEVATORS. 4. THERE WAS INSUFFICIENT ACCESS TO SOME OF THE PASSENGER ELEVATOR EQUIPMENT. SEPERATELY THE MEDICAL CENTER ALSO REQUESTED THAT THE BUTTON TO CALL THE ELEVATOR TO THE 6TH FLOOR (PENTHOUSE) BE CHANGED TO A KEYED SWITCH SO PATIENTS OR NON AUTHORIZED PERSONNEL COULD ACCESS IT. THIS SA DEFINITIZES CO-04K WHICH AUTHORIZED THE CONTRACTOR TO CORRECT THOSE ITEMS

    THIS CO AUTHORIZES THE CONTRACTOR TO CORRECT TWO ELECTRICAL ISSUES ON THE PROJECT. 1. AS A PART OF THE ELEVATOR INSPECTION THE LIGHTING LEVELS IN THE MACHINE ROOM WERE MEASURED. MANY AREAS WERE BELOW THE REQUIRED MINIMUM LEVEL OF 19 FOOTCANDLES. THIS CO ADDS LIGHTS TO THE ROOMS. 2. WHEN THE CYSTOSCOPY UNIT WAS INSTALLED IN THE ROOM IT WAS LOCATED ABOUT 36 INCHES TO THE NORTH OF THE LOCATION SHOWN ON THE EQUIPMENT AND ELECTRICAL PLANS. AS A RESULT ONE OF THE ELECTRICAL BOXES IN THE CEILING WAS IN THE WAY OF THE TRAVELING PART OF THE MACHINE. THIS CO MOVES THE BOX TO THE NORTH.
